Sky Blues in the Community are recruiting TWO NEW Health and Wellbeing Coordinators.
These roles have a wide range of responsibilities, with a principal focus on leading on the delivery and coordination of an activity on referral project in partnership with Nuneaton and Bedworth Primary Care Network.
Working as part of the Health and Wellbeing Team, the post holders will plan and deliver high quality health and wellbeing activities to a wide range of clients and participants, providing a holistic approach to personalised care for members of these local communities.

These roles have a wide range of responsibilities, with a principal focus on leading on the delivery and coordination of an activity on referral project in partnership with Nuneaton and Bedworth Primary Care Network.
Working as part of the Health and Wellbeing Team, the post holders will plan and deliver high quality health and wellbeing activities to a wide range of clients and participants, providing a holistic approach to personalised care for members of these local communities.
One post will coordinate and deliver this work in Nuneaton and the other post will coordinate and deliver this work in Bedworth. However, both roles will work together to deliver the outcomes of the project.
Responsibilities include, but are not exclusive to;
- To provide high quality Health & Wellbeing coaching, advice and support to patients within the Nuneaton and Bedworth Primary Care Network group of surgeries.
- Work as part of the wider Health & Wellbeing team with key local partners and organisations such as the Nuneaton and Bedworth PCN, Social Prescribers, GP’s and Community Connectors.
- Investigate opportunities for the development of health interventions across partnerships and with key stakeholders.
- To ensure that the SBitC health programmes support local and national strategies to improve health and reduce health inequalities within the local community. Working closely with key partners and stakeholders to ensure joined up delivery.
- To ensure that the SBitC health programmes conform to the recognised standards.
- Motivating patients through multiple sessions to identify their needs, setting goals, and support patients to achieve their personalised health and care plan objectives.
- To maximise opportunities to raise awareness of the organisations’ activities.
- To keep up to date with and understand the latest research in the area of public health, national service frameworks and physical activity.
- To carry out administrative duties as necessary e.g., maintain accurate attendance registers; monitor and evaluate assessment of sessions and carry out risk assessment of activities.
- To ensure that the safety of participants, volunteers and staff is prioritised at all times.
- Contribute towards the development of a culture of excellence and continuous improvement within the organisation.
- Comply with all charitable policies and procedures and promote the SBitC brand and ethos in a professional, strong and positive manner.
