Health and Wellbeing Coordinator (Everton Cares-CEV)
We are looking for an individual to join our expanding Everton in the Community Team as our new Everton Cares Coordinator, delivering our ‘Everton Cares’ programme across the Liverpool City Region.

About the opportunity: We are looking for an individual to join our expanding Everton in the Community Team as our new Everton Cares Coordinator, delivering our ‘Everton Cares’ programme across the Liverpool City Region.
The focus of the programme is to support those who are clinically extremely vulnerable (CEV) in various groups of people across Liverpool City Region. The programme is in collaboration with Edge Hill University and Everton in the Community’s GP, local hospitals, and specialist voluntary organisations.
The five aspects of the programme will include 1) Raising awareness 2) Completing Health Checks 3) Delivery of a social inclusion & Physical Activity Programme 4) Encouraging Peer to peer support 5) Monitoring and evaluation.
Other key responsibilities will include:
Designing, coordinating, and delivering an engaging health and wellbeing programme targeting people who are CEV or have long-term health conditions, including those residing in care homes.
Effectively engage people who are CEV or have long-term health conditions in ways that encourage their participation on EitC’s existing health & wellbeing programmes.
Liaising with Everton in the Community’s existing health and wellbeing programmes to improve our participant’s quality of life and improve their mental health.
Effectively raising awareness of ‘Everton Cares’ across the Liverpool City Region to encourage improved health & wellbeing and attendance at health check events amongst those who are CEV.
Coordination and delivery of screening events on match days and non-match days to maximise uptake and to work closely with various partners and stakeholders to ensure a seamless process with screening and follow-up for participants.
To effectively monitor and evaluate the Everton Cares programme with support from a research team from Edge Hill University.
Who we are looking for: Essentially we are looking for an individual who has knowledge of CEV, including the associated health conditions, risk factors and long-term health complications.
Our successful post holder willunderstand the importance of taking a holistic approach and providing patient-centred support.
You will need to haveexcellent interpersonal skills to build relationships with existing staff members, participants, and key external partners.
Other key Knowledge and Skills will include:
Educated to a degree level in a health-related subject.
If appropriate to degree, must hold appropriate registration to a professional body.
Experience of delivering social inclusion, physical activity, and educational activities to hard-to-reach groups.
Experience of working with people who are experiencing poor mental health.
Experience of conducting health checks for blood pressure and knowledge of clinical guidelines for diagnosing and NHS funded intervention programmes.
Experience of monitoring and evaluating community-based programmes using both quantitative and qualitative methods.
Experience of coordinating engagement events, with adults and communities.
This role will be offered on a 12 Month Fixed Term basis
