Inclusion and Transition Officer
CACT are seeking an individual to work alongside another Inclusion and Transition Officer to help coordinate, develop and deliver a programme to support young adults aged 16-25 with targeted disabilities to access universal services, including but not limited to Young Greenwich. The successful candidates will work closely with young people, their families, support services and community provisions to prepare and support them to transition from young people’ services into adult community settings.

CACT are seeking an individual to work alongside another Inclusion and Transition Officer to help coordinate, develop and deliver a programme to support young adults aged 16-25 with targeted disabilities to access universal services, including but not limited to Young Greenwich. The successful candidates will work closely with young people, their families, support services and community provisions to prepare and support them to transition from young people’ services into adult community settings.
